Google has not put my page in the index?

My website uses Google to rank high on, if there were a number of search terms. Actually went up to the first position. Then, suddenly, the main index.html page is no longer displayed. The rest of my web pages can appear for the same keywords are classified but not high. I tried using Google Webmaster Tools, and I am sure that my site is not at all in the index (ie I'm sure it will be never even appear as the millionth HIT). I tried to submit a Sitemap, but no change. The only changes I made on the front page is I call that a PHP script (as part of the from a source that IMG increments a counter and log the IP address. It is over a month, and I have done all sorts of tricks. Any ideas why this might happen? Note: The PageRank button in my Google Toolbar shows the Page Rank as 10.04.
